The knob is turned to playback

I've tried the knob straightup (halfway) to full playback. If I have it all the way to "input" I can't hear it out of the headphones. I've also set the settings to my Audiobox USB. I can record fine, playback fine, hear the playback through the headphones, just nothing out of the main output jacks, no matter where the main volume or the mix knob is set. The symptom is the same with live monitoring also. Just can't get anything to come out of the Main 1/4" outputs.

You may have a faulty AudioBox.  From what you describe, everything else seems to be working normally.

Turn your Mix Dial all the way "inputs" (LEFT)

Connect your headphones to the Main Out Left and the Right.  You will only hear audio from one side of the headphones but this is a good test for audio output.

If you do not hear any audio from Main Outs L/R, then assume your AudioBox is faulty.  Contact your store for exchange or PreSonus for repairs.

I agree with this. Specifically the part about connecting your headphones to the main outputs; that's going to tell you everything you need to know.

If you don't hear anything from your headphones when they are connected to the main outputs then it's likely a hardware malfunction.

If you DO hear something from your headphones (when they are connected to the main outs) then there's something going on with the mixer you have the AudioBox connected to.
