Why is it clipping on export at the +300 dB Level?

I was exporting an Audiobook using "export stems" with the marker set to each chapter. When it got to chapter 2 it said Clipping has occurred and that it was at +321 dB. Now of course this is ridiculous, nothing in the file was anywhere near that loud. I stopped it, tried again and then again. Each time it gave different level of clipping. On the fourth try everything looks good and it sounds fine in RX. Not sure what's going on.....

I've occasionally had something similar happen when the program is just running. It will be playing back audio give a couple of loud pops and then show this on the main mix. I had thought that this was a problem with a short or something in my XLR cord, but now that it's happened during export I'm not so sure.

 

I'm running Studio One 4.5, Presonus Studio 24c, Rockville RCM01 Mic and HP Omen Obelisk 875
