Sometimes, I'll select a section of an audio track and click "create autofade", but nothing happens. Sometimes it works, and sometimes it doesn't, and I can't figure out why. Some sections of tracks will let me create autofades, and some won't. I have tried restarting but it doesn't help.

It works reliably here on Windows using SHIFT+X.

It will only work on audio clips, not audio parts.

I was just dealing with this very frustrating bug.  It will autofade certain audio events and then all of a sudden there'll be a specific event that it refuses to fade.  What I have noticed as well is that these specific events also lack fade handles also - so you can't even fade them manually.  Usually I want to use autofades to fix clicks when editing tracks.  Here's the workaround I've been using to get around this bug.

I make the edit and bounce/glue it into a single event, with the click happening at the edit point.  Then I re-split the track at the edit point and then autofade will work/fade handles appear.  Then I can autofade which eliminates the click.
